Chris,

You can find the SNOMED CT codes for strings by searching the "Jump to" box in the ontologies classes page, in this case
   http://bioportal.bioontology.org/ontologies/SNOMEDCT/?p=classes&conceptid=root

The resulting page (http://bioportal.bioontology.org/ontologies/SNOMEDCT/?p=classes&conceptid=http%3A%2F%2Fpurl.bioontology.org%2Fontology%2FSNOMEDCT%2F444604002)

lists the various codes associated with this term; I expect the 'notation' is the field you want.

Unfortunately we do not keep the earlier SNOMED versions in BioPortal, so we can not help you with that request.
